 note = {This paper aims to report on the support for creating input forms for events and questionnaires, carried out at the Center for Institutional Research, Educational Development, and Learning Support at Ochanomizu University since around 2012. The usage of “FormGen” (Plone4 series add-on product) occupies the central part of the report. The functions of “EasyForm” developed for Plone5 series and participating the international Plone community as a Japanese translation team of “EasyForm” are also reported in the latter. As of the end of November 2020, 241 input forms used FormGen, existed. We report on online form creation support for these ad hoc activities.},
